Expanded lines of communication:
As in any relationship, the quality of that relationship is
dependent upon communication. City Line Family Medicine has
created a variety of methods to contact our office. CLFM offers
means of communication that extend beyond the antiquated
traditional phone line. The website you're visiting right now for
one. Our website has a variety of online services to enhance the
patient communication process. Need to contact us and don't have
internet access? Don't fret. Our text messaging line solves that
problem. Send us a text at the number listed at the top of the
web page to get our attention. Lastly, your doctor has the
ability to leave you a message regarding your labs through an
online patient portal described in the next bullet point. So get
with the times and help us free up our phone lines by sending us
an email or shooting off a text message.

Electronic Health Records:
In 2010, CLFM began the implementation of an
electronic health record (EHR). As our adoption process has
moved forward, many patients have realized the benefits of
electronic prescriptions and lab orders. Gone are the days of
lost prescriptions or lab slips. Now patients of CLFM, thanks to
the outstanding efforts of our EHR vendor HealthFusion, can
access their lab results online through a secure "Patient
Portal". Simply get your username and temporary password
from our office,
activate your account online by clicking the
Patient
Portal link and your ready to go. HealthFusion
continues to expand Patient Portal functionality so we're
certain more surprises lie ahead. Be proactive, activate your
patient portal account.

Quality: Although
it may not be new for CLFM - providing quality healthcare. For the third year in a
row City Line Family Medicine has once again
recognized by Keystone Health Plan East (KHPE) for
achieving a 98% score in their quality of care
measurements. In other words, CLFM's quality of care
performance exceeded 98% of the participating
providers of KHPE. Healthcare is serious business
and it starts and ends with quality.

Nutritional Counseling: Don't
have diabetes but have a need for nutritional counseling? If you
have Keystone Health Plan East (KHPE), Personal Choice (PC) or
Aetna insurance CLFM has a convenient onsite solution. These
insurances allow nutritional counseling with some restrictions
depending upon your plan. KHPE and PC members are eligible for 6
free visits. Aetna provides for unlimited visits with a co-pay
cost equivalent to a specialist visit. Ask your doctor for more
information.
