This web site does not replace medical consultation with a qualified medical professional.

This website does not provide or allow patients to directly communicate electronically with physicians.

Information submitted by using the online services feature may be viewed by any current office employee.

Do not submit any personal, sensitive, or medical information that you would not want employees to view.

Physicians will not reply to any medical question submitted by e-mail.

Submitting a request, question, or information via the websites e-mail forms that is outside of the intended use of the form will not receive a response and will be deleted. 

E-mail addresses will not be used for marketing purposes or shared with third parties.

The e-mail appointment, prescription, test result, and referral request service is not intended for emergency requests.  If appointments, prescriptions, test results, or referrals are needed within 24 to 48 hours then contact the office by telephone at 610-617-1300.

The appointment, prescription, test result, and referral e-mail request service is available only to current, already-established, patients defined as those patients who have had at least one face to face consultation with a physician of  City Line Family Medicine and currently receives medical treatment at the office.

E-mail appointment, prescription, test result, and referral requests are processed Monday through Friday from 9am - 4pm. E-mail requests sent or received after 2:00pm may not be responded to until the following business day.   Normal business days are Monday through Friday with the exception of holidays.

E-mail attachments sent will not be opened and are automatically deleted.
